Endometriosis

Endometriosis is quite seldomly seeing in the daily practice. Usually, the patients of the Endometriosis are seeking the help from the Gynaecologists. Endometriosis is endometrial-like tissues outside of uterus, leading to the lesions. Recent article (Amro B, Ramirez Aristondo ME, Alsuwaidi S, Almaamari B, Hakim Z, Tahlak M, Wattiez A, Koninckx PR. New Understanding of Diagnosis, Treatment and Prevention of Endometriosis. Int J Environ Res Public Health. 2022 May 31;19(11):6725. doi: 10.3390/ijerph19116725. PMID: 35682310; PMCID: PMC9180566.) mentioned that “Endometriosis was considered a consequence of retrograde menstruation and implantation of endometrial cells. And it was mentioned that “The biochemical variability of lesions and endometriosis in women without endometrium, the genetic-epigenetic (G-E) theory describes that endometriosis only begins after a series of cumulative G-E cellular changes.”

However, I thought that the traditional thought of the retrogarde menstruation and implantation of the endometrial cells were reasonable. Because nowadays, most of women having the periods are using the tampon. In Australia, people regularly purchase commercial products such as disposable pads and tampons, with an estimated 300 million tampons and 500 million disposable pads sold each year(Ramsay, C., Hennegan, J., Douglass, C.H. et al. Reusable period products: use and perceptions among young people in Victoria, Australia. BMC Women’s Health 23, 102 (2023). https://doi.org/10.1186/s12905-023-02197-3). That meaning is almost 37.5 percentage of the people having the periods are using the tampons.

The issue I consider is the day time that the people are standing and sitting is not a problem, due to the fact that the menstrual blood flow will downstream to make the absorption to the pad or the tampons. However, at night, during the sleeping time, when the patients are lying down, the menstrual blood is stagnated in the uterus, due to the body is horizontal with the tampons which is blocking the blood flow to outside of the body. Therefore, the menstrual blood may, can be backflowing to the fallotian tubes, and eventually spreading to the abominal cavities, leading to the implantation of the endometrial cells at the pelvioc abdominal cavities.

The conclusion is that my recommendation is not using the tampons for the periods and please use the pads all the time.

From the same reason, the intercourse during the periods is not good.

Please aviod the intercourse during the periods.

Treatment

If you already have the endometriosis, the options of conventional treatment are the surgery to remove all visible endometriosis lesions (a laparoscopy) with minimal damage, and for superficial lesions, CO2 laser vaporisation which is still theoretically superior because of the minimal tissue damage and, thus, probably postoperative adhesions.(Amro, B.; Ramirez Aristondo, M.E.; Alsuwaidi, S.; Almaamari, B.; Hakim, Z.; Tahlak, M.; Wattiez, A.; Koninckx, P.R. The problem of the laparoscopy is the postoperative adhesions which is the scar tissues at the area of surgery. the scar tissues would be pulling the local tissues, leading to be uncomfortable. The benefit of the surgery is covering the whole areas to remove the visible endometriosis lesions by one time laparoscopy.

From acupuncture point of view, acupuncture may help you to flow the menstural blood freely downstreaming without blockage, and then reducing the pain at the area of lesions by reducing the inflammation.

ACNE

Acne is quite frequently seeing at the daily practice, especially at the age of teenage and/or the young women. When I consulted with the patients, they are suffering for short times such as one year to long times such as several years. Major reason is from female hormone imbalance especially not enough estrogen. Most of the patients are complainting about heavy period with long period, for example, they might be presenting the heavy bleeding on the first and second day with more than 5 days period.

From above descriptions, the heavy period is meaning that the patients might be continuously changing the pads. If the patients are losing her blood too much through the periods, she might be losing her own blood rather than her period blood only. That is why that most of the patients are complaining about the weakness, fainting easily when she is taking the blood out for the blood test, low blood pressure, anemia, and tirednesss.

Always the early treatment is best strategy to prevent the further damage.

HOT FLUSH

Hot Flush is one of major complaints in daily practice. Most of women are approaching menopause at around 49 years old. Some women are experiencing menopause earlier than 49 years old. Certain women are having the periods even after 60 years old.

Metrorrhagia

Metrorrhagia is abnormal bleeding between regular menstrual periods. Most of patients are complaining

Dysmenorrhoea(Painful period)

Painful periods are not normal. If you get painful periods, usually you might be taking pain killers. Continuous taking pain killers is not normal. Furthermore, if you are taking more pain killers and/or more stronger pain killers, it means that your problems are getting worse.

You might be see your doctor to see what is major reason you have painful periods.

Usually in clinic, most of patients have been suffered painful periods since puberty when they had been starting periods. Depending upon how long you had been painful periods, you might be needed to check your pelvic area by ultrasound scan and blood test.

From ultrasound scan and/or blood tests, you might be not any particular reason you concern.

It might be coming from muscle spasms in pelvic area.

Most of patients are complaining period pain before periods and/or during early periods. When and/or before periods, your body would like to be ready to periods. Uterine membrane is getting swollen up with dilation of blood vessel in uterine membrane by responding female hormone change. Therefore, dilations of blood vessels are getting affecting nerves surrounding blood vessels, leading to jam nerves. Eventually, you would get pain. Those pain are leading to cramp muscles such as uterus muscle layer and/or colon muscle layer, furthermore, abdominal muscles.
